Advanced Nurse Practitioner (ANP)

Our Advanced Nurse Practitioner ANP has specialist skills and experience. She runs her own clinics and can offer assessment and advice on minor illnesses, minor injuries, home care, and medication available from the pharmacy without a prescription, and a wide range of other medical issues. Nurse practitioners are qualified experienced nurses, educated up to a Masters degree level, who work as independent, autonomous practitioners. Nurse practitioner training enables them to: • Perform an in-depth clinical physical examination • Initiate investigations where necessary • Reach a diagnosis • Advise on treatments and where necessary prescribe medication for acute problems • Refer to other members of the healthcare team where appropriate or necessary. The Advanced Nurse Practitioner are available daily as part of clinician call back and by appointment on request. Advanced Nurse Practitioners are able to prescribe independently although a Doctor is always available to review you if the Advanced Nurse Practitioner feels it is necessary. Our surgery is currently recruiting a further experienced Advanced Nurse Practitioner to join our clinical team.

Nurses

Our surgery currently has three Nurses, one Nurse Associate and a Health Care Assistant who provide professional support and advice in a variety of areas. Our nursing team are highly qualified and provide a variety of general and specialist nursing services (as listed below) and are responsible for the management of most chronic disease clinics. They are available 5-days per week via appointment. Our Healthcare assistant supports our practice nurses with their daily work and carries out tasks such as phlebotomy (drawing blood), blood pressure measurement and new patient checks. They may act as a chaperone when a patient or doctor requests one.

Pharmacy Team

Our clinical pharmacists and technician are experts in medication issues. They are involved in medication reviews, answering queries you may have about your medication, making sure you are on the right medications after a stay in hospital and liaising with local pharmacies. In-house pharmacists are becoming more and more common as part of the team of clinicians who are working on our patients’ behalf. Because they are experts in their field, medication issues are often best placed to be dealt with by a pharmacist, rather than a GP. They undertake medication reviews.

Practice Management

We have a management team of two who are involved in managing all the business aspects of the practice, such as making sure that the right systems are in place to provide a high quality of patient care, human resources, finance, patient safety, premises and equipment and information technology. They support the GPs and other medical professionals with delivering patient services and help to enhance patient care. They would be happy to hear your views and suggestions about the services offered by the surgery.

Tracey joined the practice on July 1st 2021 as Head of Primary Care at Tieve Tara Medical Centre. Tracey has over 35 years' experience working in the NHS as a healthcare professional. She qualified from Leeds school of Physiotherapy in 1987.

Julie recently joined the practice on September 18th 2023, as Practice Manager at Tieve Tara Medical Centre. Julie has over 28 years’ experience working in the NHS in various roles and NHS organisations such the Ambulance Service, NHS England and the last 6 years in Primary Care settings.

Reception, Administration & Secretarial Team

Our reception, administration, and secretarial team of nine provide an important link for patients with the surgery and are your initial contact point for general enquiries. They can provide basic information on services and care navigate you to the right person depending on your health issue or query.

Our Receptionists

Our Receptionists – have a varied roll. • They make routine appointments with the GPs and nurses. • They can deal with you face-to face 08:00 – 18:30 Monday - Friday or over the telephone between the hours of 08:00 – 12:00, 13:00 - 18:00 Monday - Friday. • They also are responsible for giving out test results to Patients and remind Patients of any repeat tests that you may require. • They deal with all the repeat prescriptions that you need, including on-line requests and remind patients when a medication review is due. • Register all new patients.

Our Administration team

Deal with a wide variety of paperwork that comes into the Practice. They handle requests from solicitors for medical records and medical reports for employers as well as the processing of all clinical letters from hospitals and other places of care. They also co-ordinate the chronic disease review clinics.

Our Secretarial team

Deal with all general correspondence between patients and the doctors as well as referrals to consultants (both private and NHS). They also deal with general enquiries relating to 'E-Referral Service' (ERS) formerly known as ‘Choose and Book’ appointment service and chase up any letters that a doctor may need in relation to your care. The secretaries also help book, cancel and re-arrange appointments should you need any help. All these jobs are very demanding so please try and be patient. Once again, all information is treated in absolute confidence.

Primary Care Multidisciplinary Health Care Team

Sometimes the GP isn’t really the best person to see. Patients can be seen and treated quicker by a nurse or a physiotherapist for example and in some cases, the GP practice might not be the right place at all for the query. Community Midwifes

There is a team of midwives who will give antenatal, intranatal and postnatal care to all pregnant women who are registered with the surgery. To make an appointment with the midwife please contact the surgery. Midwifes contact number is Pontefract 01977 747432 Wakefield 01924 543424.

Community Nurses

The community nursing team is based at Castleford Health Centre and can be contacted directly (without having to telephone the surgery) by calling 01924 327591, 7 days a week, 24 hours a day. Community nurses or district nurses visit people in their own homes or in residential care homes, providing care for patients and supporting family members. They provide holistic care for all adults refereed to the service, such as terminal care, wound care, post-operative discharge follow-ups, administering injections, undertaking CHC assessments, incontinence assessment and ear syringing. They also have a teaching role in that they work with patients to enable them to care for themselves. A vital role is also played in keeping hospital admissions to a minimum and ensuring that patients can return to their own homes as soon as possible.

Health Visitors

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
